I am 29 years old and I am married. When I have sex with my wife, I am unable to ejaculate inside of her. However, when I masturbate, I ejaculate just fine. What is the issue?

When ejaculation is difficult, not occurring or happening only after a lot of effort and after much time has lapsed during intercourse, it is called ‘delayed or retarded ejaculation’. It is not very uncommon and seen in men of age groups. It may either be a biological (organic) or a psychogenic problem. It is also known to occur temporarily due to situational causes. Biological causes include – diabetes, nerve damage, vascular diseases, hormonal imbalance, surgical procedures such as bladder or prostate surgery and alcohol or other drug use. It also occurs as a side effect of some medications such as certain anti-depressants (SSRIs) and antihypertensives (Alpha Blockers). Psychogenic causes include anxiety, stress, guilt, anger, mood disorders, relationship problems and past traumatic experiences. In the cases of psychogenic or situational causes, the treatment is usually ‘sex counselling’ and ‘supportive therapy’. If the cause is biological, then appropriate medical or surgical measures need to be taken. It is always necessary first to ascertain the cause (diagnosis) of this problem through proper history taking, thorough physical examination and certain investigations. Once the cause is found, only then can treatment be advised. I recommend you to personally consult a qualified sexologist for your problem.
